When Copilots Fail: The Risks of Overreliance on AI

The rise of AI copilots in business has transformed the way we work. In just a few years, AI has progressed to drafting reports, summarizing data, flagging risks and even influencing business decisions. The idea of a “copilot” is compelling: an AI assistant that handles heavy lifting while we remain in control. But what happens when these AI assistants fail?

Failures don’t always come from the technology itself. More often, they stem from user willingness to hand over responsibility without asking the hard questions. In high-stakes industries such as finance, law, healthcare and compliance, that overreliance on AI escalates into costly mistakes, regulatory breaches or a loss of trust.

Misplaced Trust: When AI Copilots Fail in High-Stakes Industries

J.P. Morgan Chase's head of data and analytics, Tiffany Perkins-Munn, has seen firsthand the dangers of this misplaced trust. "The biggest mistakes I’ve seen aren’t AI failures themselves, but an overreliance on AI in complex, high-stakes environments where a single error can have massive financial, legal or ethical consequences,” she told Reworked.

For example, take a compliance team that depends on AI to flag fraudulent transactions. On the surface, it’s efficient, because AI assistants sift through large amounts of historical data and highlight anomalies faster than any human team. But fraudsters evolve, and AI trained on the past might miss the new tricks. Without human judgment to interrogate the model’s assumptions, the organization risks a compliance breach. This pattern repeats across industries. A perfectly formatted legal summary or a polished compliance report may still be wrong. Guillermo Carreras of BairesDev described how this misplaced confidence can play out in real life: auto-approvals for refunds or access rights pushed through without checks, compliance decisions made on faulty summaries, or sensitive data slipping into prompts and resurfacing elsewhere.

Each of these failures has the same root cause: AI’s polish masks its errors, and no one feels accountable enough to intervene.

Warning Signs of Overreliance on AI

Danger signs are easy to miss because AI overreliance creeps in. Perkins-Munn identifies the absence of critical questioning as one sign of creep. When teams stop interrogating outputs, they start slipping into “AI said so” thinking, she said. Carreras adds another: when the audit trail stops at a bot. If no one can identify who owns a decision, the organization is already in trouble.

Warning signs are not only in process but in people, said Louisa Loran, a global executive advisor and technology consultant. When employees sense that AI has taken over decision-making, they may begin to feel unnecessary, and overreliance leads to disengagement. “This sense of not being needed has severe psychological effects,” she warned. Productivity, motivation and engagement drop — risks not just to decision quality, but to workforce health.

There’s a cultural shift from “trust and verify” to “trust fully,” agreed Salable co-founder Neal Riley. As AI assistants improve, organizations fall into the trap of assuming AI oversight is no longer necessary. When verification disappears, small mistakes are free to multiply.

Not all tasks carry the same risk. Failures hit hardest in roles that deal with nuance, ambiguity and ethics. Compliance, risk management and legal services are especially vulnerable, Perkins-Munn said. Carreras added healthcare, where flawed data or misdiagnoses have life-or-death implications.

Even outside traditionally high-stakes sectors, AI copilots in business can fail in subtler but equally damaging ways, Loran said. When companies use generic automation for processes that define their unique value, such as customer relationships or innovation, they risk commoditizing what sets them apart. The failure here isn’t a lawsuit; it’s the slow erosion of competitive advantage.

High-Risk Tasks Humans Should Never Give to AI Assistants

Across all the analysts we contacted, a strong consensus emerges: Certain domains cannot safely be left to AI. These include:

  • Ethical judgment and complex dilemmas, which require empathy and contextual reasoning.
  • Regulatory and legal attestations, where accuracy must be ironclad.
  • Medical diagnoses and treatment decisions, which carry human consequences too great to risk.
  • Hiring and firing, where fairness, privacy and human dignity are at stake.
  • Strategic storytelling, innovation and original thought — areas where human imagination still outpaces machines.

“These are the unique skills that make us human, and lucky for us, AI can’t replicate them yet,” Perkins-Munn said. 

How to Prevent AI Failures

The solution isn’t to sideline AI. Its speed and efficiency are too valuable to ignore. Instead, organizations must redesign their workflows so copilots don’t become pilots.

Carreras recommended scaling oversight by risk: Let AI run low-risk tasks end to end, apply sampled human review to medium-risk work and keep high-risk decisions firmly in human hands.

Equally important is building AI literacy across teams. Literacy is less about coding skills and more about curiosity and skepticism: knowing where AI is strong (pattern recognition, data crunching) and where it’s weak (edge cases, context, ethics), Perkins-Munn said. 

 “Verifying and vetting its output remains essential,” Riley added. “Without active training, employees risk using AI as little more than a glorified search engine or blindly automating tasks that don’t benefit from automation at all.”

Infrastructure also plays a role. When multiple AI agents overlap or conflict, IT teams are left to referee, wasting resources, said Tray.ai CEO and co-founder Rich Waldron. Without orchestration, governance and integration across systems, copilots can fail simply by working at cross purposes.

AI Overreliance Isn't Inevitable: Keep Humans Behind the Wheel

The future of AI copilots is not about replacement but collaboration. Perkins-Munn envisions a world where AI assistants accelerate analysis but humans apply context, empathy and judgment. Loran sees the most successful leaders as those comfortable inviting AI into their work without feeling threatened, striking a balance between skepticism and openness. AI systems themselves will evolve to better explain their rationale and uncertainty, including humans in the loop rather than excluding them, Riley said

The key is accountability, Carreras said. “While AI becomes the first drafter and fastest analyst, humans remain the accountable decision-makers,” he said. That accountability, notably clear ownership of every decision, is what will separate teams that thrive from those that stumble.

The most insidious risk of all may be the erosion of critical thinking. Perkins-Munn said she has seen people treat AI output “as if it’s gospel instead of a starting point," which is when organizations end up with bad decisions, compliance headaches or public-facing mistakes that cost trust.

Ultimately, AI assistants fail not because they are inherently unreliable, but because humans forget their role in the relationship: to guide, question and apply judgment. When organizations drift into “because AI said so” logic, they hand over the wheel. And in high-stakes environments, that’s when crashes happen.

Learning Opportunities

The real test of the AI era won’t be how powerful copilots become, but how well we resist the temptation to let them fly the plane alone. Companies that succeed will be the ones that keep human judgment front and center, using AI for speed and scale, but always remembering who holds responsibility for the journey.
